Customized form on bug closer

I would like to know if its is possible to have a feedback form on bug closer.
If yes how that can be done?

You need to be more specific about your request. Who fills in this form?
Is it the bug reporter, the assignee, or someone else? Where would you
like this form to appear, given that the person closing the bug could be
someone else again?

Form should be filled by bug reporter, when he/she tries to close the bug.
I understand anyone can close bug but can we make it compulsory that reporter only close the bug.

The feedback will be in form of questions and multiple choice options.
This should be stored somewhere either in tables so we can study those later. I will go through the links you provided and see how to implement this.
